Easton Family Practice and Seymour Medical Practice contractually merged on 1 April 2015.
On Monday 29 June 2015, we will merge our patient services and will be serving all our patients as Charlotte Keel Medical Practice. Patients can continue to contact us using the telephone number they previously used and the GPs, Nurses and Health Care Assistants from both practices will all be part of the new team.
To allow our computer systems to be merged, we will not have access to our clinical systems from 1pm on Friday 26 June 2015. From 1pm to 6.30pm on that day, our GPs will continue to provide urgent healthcare to our patients although they will be working without access to your clinical notes. This system downtime may also have an impact on the time taken to process your prescription requests so we would ask you to allow an additional day when requesting your repeat prescription.
We will be launching a new web-site next week and you will then be automatically diverted to the new site when you access this site.
Patients previously registered with Easton Family Practice to use Patient Access for online appointment booking or prescription ordering will need to be re-registered for this service and we will be contacting those patients early next week to arrange this.
